Independent RNs: please email your resume to provider-training@longtermsol.com before completing the field assessor enrollment information.Current Field Assessors do not need to resubmit resume.

Once your resume has been received and reviewed, you will be contacted by an LTS representative if we have a need for field assessors in your area.

Upon approval by an LTS representative to begin the Field Assessor Network enrollment process, please fill out and submit the Field Assessor information below.
