  • Comprehensive Assessment

Field clinicians complete objective assessments on long-term care insurance claimants.  LTS provides accurate and timely documentation essential for insurers to determine claimant eligibility.

  • Plan of Care Development

Field clinicians work with the LTS in-house nursing team to create a plan of care that addresses individual claimant needs. Based on these needs, the comprehensive plan of care recommends community resources, formal and informal caregiver support, and medical equipment.

  • Care Coordination

LTS clinicians continually review cases to ensure that claimants receive the appropriate level of care prescribed.

  • Care Delivery Services

Our field clinicians evaluate and train caregivers to ensure that selected paraprofessionals provide appropriate care to the client.  Clinicians also visit facilities to evaluate the quality of care delivered.

  • Medical Information Retrieval

LTS staff gathers and maintains accurate and complete medical information to help in the adjudication of claims.

  • Telephone Applications

Our trained staff interviews applicants via telephone to obtain thorough medical history. All interviewers are trained to probe for critical information needed by underwriters. We focus on customer service and expediency.
 

  • Telephone Assessments

LTS partners with insurers to assess applicants using a telephone assessment tool, a tool that provides information to meet underwriting needs. Trained interviewers complete a full health and cognitive assessment which provides thorough information and allows the underwriting department to make eligibility decisions.

  • Face to Face Assessments

Skilled clinicians visit candidates in their homes and do a comprehensive functional, cognitive, medical and psychosocial history to evaluate eligibility. The information is sent for review to the appropriate department within the insurance company.

  • APS Summaries

Skilled clinicians review medical records and create a summary to assist underwriters in making decisions.
