Our WeCare+ service offers five components of care for your loved one:
- WeCare+ Assessment
- A comprehensive needs and safety assessment is conducted by an experienced clinician
- WeCare+ Plan of Care and Provider Referrals
- Our clinician develops an individualized plan of care based on the assessment
- Our clinician offers a list of qualified local Care Providers from the LTS Select Provider Network to meet the client’s needs
- Each provider referral includes quality reports
- LTS negotiates a discount for clients who choose a participating Select Provider
- WeCare+ Care Coordination
- Our clinician experienced and specialized in geriatric care communicates with the family to ensure that the plan of care continues to meet the client’s needs
- WeCare+ Monitoring
- Premium service includes on-going care monitoring and coordination for the duration of the subscription
- WeCare+ Communication
- Access to the WeCare+ Talk secure web portal for easy communication between family members, caregivers, friends and medical personnel
- Includes plan of Care, referrals, care coordination and access to personal Care Coordinator
