Comprehensive Assessment
At Long Term Solutions, Inc we offer an initial 30 minute consultation.
During this initial phone consultation a Care Coordinator will conduct a comprehensive assessment to ascertain the needs of the client.

If needed, a field clinician can then visit the client in person to more clearly understand the care needs.

  

Care Plan Development
A care plan will be developed which incorporates the needs of the client and utilizes appropriate, screened providers.  Discounted provider networks can be accessed to attain the lowest cost possible when appropriate.

Care Coordination
Care Coordination offers an ongoing monitoring and reassessment of client’s needs and physical status if needed or desired.
